First guy said they don't have application forms. Second guy said the only way is online and physical visit to embassy is not required to complete the process.

Report Check-InOpen 9-15 h Mon-Thu, 9-13 h Fri. Multiple entry visa valid 4 months with 1 month maximum each individual stay for CFA 70k now. The guy still likes joking around. He asked for one passport photo, hotel confirmation and copies of passport, Cotê d'Ivoire visa and yellow fever vaccination.
Applied after 14 h and could pick it up the next morning. They may be able to do it the same day if you arrive in the morning and if they're not too busy.
We must have had the same guy behind the desk because he was joking around all the time. Really helpful staff but it was quite busy (probably because of the holidays) so we got our visa in 2 days.
It costs 50 000 CFA for single entry visa.
We needed a copy of the passports, and a copy of the Ivory Coast visa, and the copy of the yellow fever booklet.
3 photos, copies of passport, visa of ivory coast and if existing old guinean visa. 50000cfa for 1 month single entry. I applied 1pm and got it 2pm the next day.
The guy behind the desk may joke around with you (told me this was the embassy of Mali), but he seems to be correct.
